Info and Procedure to Port Landline to non-Verizon cell carrier

Friends,

I'm done with Vz continually raising my landline rates after I cancelled DSL and want to port my number to a cell phone. (I've had the number for 39 years, and it has been in my apartment for 45 years!)

How do I:

a) port the number to a cell phone I already have, and

b) then cancel the service and say goodbye to Verizon forever?
