My elderly grandfather doesn’t always hear my grandmother when she has fallen or need help because the Tv is so loud. Is there a way to lock it so it doesn’t go above a certain level?

The only way I can think of is to take advantage of the volume control built into the Fios box. You would have to go here and set the volume to a very low value to limit the maximum:
Main Menu > Settings > Audio & Video > Audio>Fios TV One Volume.
You set it low enough that no matter how loud he makes the TV, It won't go louder than you want.
