Lost phone at Fort Worth Verizon return center

I returned an i14 to the Fort Worth facility using original shipping box and return label provided by Verizon.

UPS tracker indicates package received by Verizon Fort Worth Return facility

Verizon tells me that the box was returned empty.

Is there a theft problem at Verizon Fort Worth? What to do??

How do I get the phone off my account and more importantly not be charged for the phone?
