Verizon Offer to pay off for my previous phone after switch to Verizon

Hi, where can I submit my final bill from my previous carrier so that I can get the rebate to pay off my device. I switched to Verizon on Apr 4th and submitted my order online. I'm looking for pay off only for one phone. I have the bill ready but not able to find the place where I can submit it.

Have you tried using a computer to login to your My Verizon account and seeing if there is something under the rebate offers or your order history to upload the bill?
