I signed up September 8 and have yet to receive info on redemption of the Amazon gift card. Is there somewhere I should receive it or something I need to do?

Visit vzw.com/rebate.
Enter the purchase date and promo code: 250AMZFWADIG
Enter the appropriate information: Email address, phone number assigned to the new 5G Home Equipment (you can find this under account details on the My Verizon app or vzw.com/myverizon), First/Last Name.
Submit the offer redemption within 60 days of activation.
You will receive the Amazon Gift Card via email within 8 weeks of submission.
