Disconnected account / final bill

Hello,

I'm desperately seeking a way to get my final bill from Verizon - the disconnected account login form does not work, and the nearest corporate store is unable to help me without my account number which…I do not have and do not have a way to get. I'm hoping that by reaching out here someone can help.
