The device unlocking policy just changed, from what I saw yesterday in the forum announcements. It used to be that you had to use the phone for 60 days on Verizon, and then it would automatically unlock. But now it says only devices purchased through Verizon and paid off will unlock for postpaid, and if you're prepaid, you have to buy the device through Verizon and use it for a full year (365 consecutive days on Verizon) with no payment history problems.
If you bought the phone from some other retailer prior to this policy change, I don't know if you can still request an unlock if it met the previous policy requirements. Have heard of instances where a Verizon variant device wouldn't ever unlock, probably because it was purchased outside of Verizon. In that situation, assuming you're the original owner, you'd need plenty of proof of legitimate purchase and be the original owner to have any luck with a SIM lock appeal.
