Switching # from T-mobile to Verizon?

This Christmas I received an iPhone 5 that is already connected to the Verizon network and I want to have the phone number from my current phone switched with the one that came with the iPhone. However, the phone I currently use is connected to the T-mobile network. What is the procedure I need to follow in order to switch the number from my current phone/network to the iPhone?

You can port over your T-Mobile number onto that line associated with the iPhone 5. Call Verizon Wireless customer service at 1-800-922-0204 for assistance. Your line of service with T-Mobile will be cancelled as a result of the port process. If you are under contract with T-Mobile, you will be subject to their ETF.
