How do I get battery replacement with warranty?

When I purchased my Galaxy Note II in July 2013, I also paid for the 2-year protection plan that includes one free battery replacement.  I would like to get the battery now but I can't find the paperwork that I got when I bought the phone.  Can I just use my phone number/account or phone serial number to confirm my warranty, and whom do I contact about it?

That sounds like an amazing protection plan to have! Where did you purchase it from? None of our protection plans offer a battery replacement.
