If you're unable to find your PUK via your My Verizon account, you'll need to contact Customer Service by phone at 800-922-0204 (assuming you have access to another phone) or by chat (using the Contact Us link at the top of the page).
When you're logged into My Verizon, navigate to My Devices > Device Overview. On the right-hand side of the screen under About My Device, click on My PIN and unblocking key (PUK).
It doesn't say any of that, there's nothing on the right side of the screen.
