Volume for ringing on incoming calls starts loud then fades to were sometimes I don't even know my phone is ringing.

When I am receiving an incoming call, the volume on the ring tone starts out loud but then goes real low where if I do not hear it on the first couple rings I can not hear it ring at all and miss calls. I have the ring volume to Max.?

Try going to Settings-Gestures and then check that Fade Out Ringtone and Silence incoming calls (by flipping phone) are NOT checked.
