My Verizon Note 4 just warned me that my software has been modified and my phone may be a risk. I can't find any information on this and when I did a scan, nothing is found. What can I do?

Yes it did.
The app is suspect. I see a lot of these "warnings" instigated by that app. It is Verizon's way of saying, "Stay with the program and keep this feature." It is basically a pop up advertisement for itself. Nothing can modify your system without access to system files which is not possible unless some rather complicated measures are taken first by someone who really knows what they are doing, and that person would have to be the user as doing so remotely is rather elaborate. I know that app comes standard and is also a part of Total Equipment/Mobile Protection with the $11+ package with insurance and all, but even the free version uses this pop up and you can do yourself one better by just using Avast, AVG, 360, or some other antivirus app or CCleaner for Android (like the Windows desktop version) or use nothing at all since viruses in Android are about impossible to get without some serious effort. As long as you don't install apps from unknown sources, don't debug or user developer options, you should be fine.
