Has anyone else had a problem with the volume buttons and programmable key not working? I have reset my phone a dozen times with no change.

Sounds like it is a hardware problem, maybe the contact points for the buttons to the internal phone parts. A reset is really only good for software problems. Do you still have warranty? Do you have insurance?
