HOW DO I FIND MY PUK CODE ON MY HTCM9
We always want you to be able to easily manage the device, RSTRAP61. You can access the PUK by logging in via http://www.vzw.com/myverizon and click Manage My Device. YOu will be presented the option to generate a PUK.
