How do I file a formal complaint against a customer service rep?

I need to file a formal complaint, too. I'm very frustrated after my last call and the person inability to keep a consistent answer or insure me of an immediate resolution.
I need a number to call or an email to utilize.
We want to do all that we can to deliver you the best possible service, Aliciareview. Whether it’s customer service or cellular service, we are receptive to your feedback as this makes us a better company. To be sure, let’s make sure we wrap our arms around what happened. What was the reason for contacting customer service? What was the resolution expectation set by the rep?

As I remember, the only reason I called was because my credit card that was
on autopay was compromised, and I was not aware until I was overdue and
received my bill which was double. I knew I needed to fix the autopay and
make an immediate payment. Then while I was at it, I figured I would as for
the late charge to be removed since it's not common for my account to go in
overdue status.
