How do I set up voicemail on my IPhone 6s?

I have attempted to set up voicemail on my IPhone 6s, but it fails leaving me with "setting up voicemail" , and going nowhere. Any instructions?

Are you setting it up by selecting the phone icon and then voicemail? Or by dialing *86? If setting it up via the phone icon are you connected to the data network or WIFI network?
