Dates I switched devices

ok my phone broke and in the mean time I switched devices I had a 6 and switched number to a 5 that was on my plan as well but suspended now the insurance is saying that the phone wasnt on the plan on the date I gave em but idk the date so I need to know the date I switched devices on my current number any one know how ?????
