Contract Buyout claim form not working

I received my final bill from XFinity and followed the link that was included in my 5G Home Internet Welcome email to upload so that I can be reimbursed for my early termination fee.  The page loads fine and I can see where I am supposed to fill out the information to submit, but most of the fields are grayed out and will not let me enter info.  The only fields that allow input are "termination fee amount" and "previous provider."  I cannot enter my name, address or email address.  I also cannot upload my documentation or submit.  I have tried several times to speak with customer service but they seem to have no idea what I am talking about.  I am specifically talking about this page:

Not surprised customer service is not responding.  They're not helpful when I call or text either.  I have been a Verizon customer for less than 2 weeks and I've already had more bad customer service experiences than any company I've ever dealt with.  Maybe it's time to rethink switching providers.

I have the same issue. The form is not working. I talked with 3 different agents and Supervisor, the supervisor hung up on me. These people are clueless and i have wasted 6 hours of my time. I need help with this by someone who actually knows something. Thanks
