The website to upload my final bill showing the early termination fee from my previous internet provider does not work. I have posted the link below the name and address sections are unable to be filled and my pdf will not upload this looks like it has been an issue for well over a year. How has this not been corrected yet? This seems like a clear case of fraud on Verizon’s part

I have the same issue. After having trouble with a form from a link that the sales rep sent to me, I spent an hour on the phone today with tech support only to be sent a link to this dysfunctional website. This process seems to be designed to discourage customers from processing this fee buyout. Surely feels fraudulent.

We're sorry to read that you have not been able to redeem the offer for this buyout. What happens when you try to access the link? ~Peter

I can access the page, but I can only enter text into the Amount, Provider, and File fields. I cannot enter text into the other fields nor can I submit. I have tried using my phone and PC, as well as two different Browsers.
