Submitting Early Termination Fee Bill from Previous Internet Provider

I keep trying to submit my bill from my previous internet provider showing my early termination fees and so far have not been able to. The boxes are greyed out for most of my information and everything I have tried to upload will not work. It tells me that file cannot be uploaded. I have tried PDF, JPEG, etc. as well as one page upload vs. each page upload. I need to get this submitted so that I can get that credit. Is there another way to submit this information?

I was able to take it in to a corporate store and they helped get it uploaded for me. It was an easy process and I appreciate all of their help.

Trying another browser gives me the same error and same greyed out boxes.
