Verizon forward issue

I applied a month ago and was qualified. I went to sign up today and could not check out. Then got an email saying discount was not approved this time. 

How do I reapply? What is going on?

I do meet the requirements. I went through the qualification process. It took a couple of days from first applying and it being "pending" to "eligible".

The email gives zero reasons.
