Accidentally locked my sim card, new the PUK someone please help
I am not sure what your post is referencing to but I guess that this is the area that you are talking about... If you hover your pointer over the "My Verizon" option you should see the option or you can click at https://nbillpay.verizonwireless.com/vzw/secure/router.action while you are logged into your My Verizon accounts.
I think you can get it a new PUK code through your MyVerizon account online.
Zcarter125, I have locked my sim card before. In order to resolve your issue can you please DM me your mobile telephone number? I will reset your PUK code for you. Thanks.

I'm also having a similar issues and need the PUK code, I called Verizon's phone support but they said they were unable to help me with the PUK code for the sim card and told me the sim card wasn't necessary for the phone's usage.
Sammuel if you could help me retrieve my PUK code I would greatly appreciate it.

Thank you for the post! I would be happy to reset the PUK code for you. On 3G devices, you only need the SIM unlocked for international use. This is why they may have told you the card was unnecessary. Please send me a DM with your mobile number, and I will get this taken care of for you!

A "DM" means a "direct message". You have to follow a person and then they have to follow you before you can direct message each other. And please don't post your phone number in a public forum. The entire world can see it now.

I deleted your previous post since it contained your mobile number. I am happy to help you with your SIM card. I would like make sure I have all of the information first. Will you be traveling internationally? The only time you need a SIM unlocked for an iPhone, is if you will be traveling to a country that does not support CDMA technology (Canada and Mexico do not require a SIM for example).

Where is this device actions page that it's talking about? I'm on the MyVerizon homepage for my cell phone and I don't see any "Device Actions".
There is just no area for "device actions" under My device. I've searched it a few times, post a ss of the area where its located?

To get a hold of one of the Wireless Employee that's trying to Assist in helping answer a Question or solve an Issue you Need to Put your Mouse on there Screen Name, Or this Box shown Here when you do that it Brings Up a Bigger Box as you See this slide your Mouse down to were it says Follow Click it. You will see a big blue Bar towards the Bottom that says waiting approval Meaning the party you want to follow has to Approve to let you follow them. Now Once they give the Approval they will send one back to you An you will see it in the Actions Box Look up just a little it's Red with a Check in it. Go in there you will see these Three choices Look in your Notifications for new Followers that want to Follow you will have {Accept} {Reject} {Ignore} For your Choices: To let them follow you an then send a D.M. = Direct Message you will need to Check Accept then you a Can send your Private Messages or Cell phone Information instead of Posting it on the Main Post were other Eye's can Read it. Sending a D.M. is Easy, It's Fast, An It's Safe. B33 :smileyhappy:

When I log into My Verizon, I can see the option to unlock sim under device actions if I click on "Device" under "I Want To..." (it will show options for Bill, Plan, Device, and Profile. Click on the word "device" instead of one of the options under it). This should show next to the picture of your device and under your upgrade eligibility info. I'll admit that I can't recall if the same option appeared when my iPhone 4s was still on my account but it does show with my iPhone 5 on the account.
Just realized that I responded to an older post because I assumed the last post on the last page was the most recent post. It was not.
That option is not there
Again, since this is the 4s forum it's possible that the option doesn't exist...I noticed in the images provider in an earlier reply it shows as info for locating the 4G Sim Pin/PUK...which may explain why I can see it for my iPhone 5 but can't recall seeing the option for my 4s.

thanks for responding. I went to my verizon however I don't see
anything and I have two tries left. I have the wallet card that came
with the phone but more than 8 numbers on each. Then it ask for a new
pin. The new pin I read was default at 1111 when entered twice it
failed and left with two tries.

Sorry but you would have to take it to the Verizon store and or call
customer service if you have a land line. When I took mine I had no
land line and they had to reset it from store while on the phone with
tech support or phone support.

Usually when you inputted the wrong PUK 4 time the card is no longer useable and you have to get a new SIM..

Log into your on-line MyVerizon account; at the top of the page where it says MyVerizon, hover over that and click on View All. Assuming you are logged in as the Account Owner, choose the appropriate device at the top of the page. Under About My Device, click on My PIN and Unlocking Key (PUK), and you should get a pop-up with the info. If that doesn't work, contact Customer Service.
