My son recently got a iphone XR. He is on a college campus and is connected to wifi 24/7. His phone is still using data while he is connected to wifi. His background refresh is off. His wifi assist is off. Why is his phone still using data?

It's important to close all applications and Internet pages when he's done using the phone to avoid data usage. Maybe because he hasn't manually turned off the cellular data connection or set the application preferences to not use cellular data.
Just because the wifi is turned on and connected doesn't mean it is always active. The iPhones have a Wi-Fi sleep policy where it shut off the Wi-Fi when the phone is idle.
