Charged after service was cancelled

So I decide to move my service from TMobile to Verizon because the service with my internet and cable has been so excellent. The wireless account is set up and the new phones sent out for delivery. Someone retrieved the phones and signed my name with the UPS driver (I have concerns on that end as well). In any case I never got the phones. I reported it to Verizon immediately and was told they had to open an investigation. The signature was provided by UPS and it obviously didn't match mine. I offered to provide my signature to Verizon and was told it didn't matter.  All this talk about an investigation but  nothing was done. As a result I cancelled my service and stayed with TMobile. In the meantime Verizon still had the $132 they charged for the taxes on the phone....and then, they pulled the first month's payment from my bank account ($302.79). Now, remember I had requested the account be closed weeks before.  I realize the bank surprise and called Verizon about the refund. Guess what? Another investigation has to be opened.  One month later nothing. I've called several times over the past few weeks to get an update or some explanation and nothing. "I'm not sure" "I can't tell" aren't good excuses. I got an attorney to file a small claims case but there is a form that has to completed online. Guess what? I asked for your account number. Well, once the account is closed the number goes into the clouds so the electronic questionnaire is useless. There is lots of feedback about the service and how good it is - for cable and internet I totally agree. On the wireless side there is no service. Everyone lives in a paper bag. I will get my money one way or the other. If a Verizon rep is reading this step up and do your job. Help your customers.
