Over charged

I am curious why my phone bill is 40$ more than my plan. My phone is paid for as well. My plan is 93$ a month. Yet, my bills are usually 133$.I went to look at an itemized bill, and it's not there. I had to request it. What is going on? I have been getting completely ripped off, in my opinion. I need someone to explain why this is happening and what I am paying for. I
