Still being charged months after switching to Visible

I switched to Visible in October. I was under the impression that I disconnected each of my devices (phone, iPad, and Apple watch) from my Verizon account (and each customer service rep I speak to has verified that they're disconnected), but I am still being charged (this morning for a grand total of $82.74). No customer service representative can figure out why this is happening, neither can any Verizon store employee. I can't log into My Verizon because my account is locked (due to it being disconnected). I am about to lose my mind, respectfully. Please advise.
