Orders for Installation of lindline phone not found twice

I'm so distressed! I've requested a triple play twice. East time, when I check on it and after 90 or more minutes with an agent I'm told. "We can't find this order. We must have you place a new order" This after waiting 3.5 weeks for an installation date.
Has this happened to anyone else? How did you resolve it?
