Costco $250 shop card rebate

I recently switched from T-mobile to Verizon having been a long time T-mobile customer getting the new lines at a Costco kiosk to leverage the Costco Shop card and the Verizon carrier switching offers. I already submitted for a $200 rebate that was for switching wireless providers from others into Verizon. What is the correct procedure to apply for the $250 Costco shop card offer?

Thanks for reaching out with this great question. All you have to do is upgrade or add the line of service. Reporting is sent verifying the line has been active for a minimum of 90 days. Costco then sends out the Digital Shop Card by email to you (The Digital Shop Card email takes up to 14 weeks to deliver. The gift card does not order until after the customer hits the 90 day time frame for the Costco Return Policy). You receive the cost co email from costco@digitalshopcard.costco.com and follow the instructions to Active Card Now.

Call Costco Customer Care at 888-668-9668. Have your membership number and email used for Costco ( I assume ) and they look up the Shop card then reissue another one. Had mine within minutes of calling them and only spent a couple minutes once I had the customer care person.
