We have the Hotspot and pay $20 monthly fee. My question is will it work in Italy without any additional expense? Will be charged a roaming fee? If so can we buy a foreign sim card to avoid?

Going to Italy sounds very exciting. I hope you have a great time. Options to use your device in another country vary a great deal depending on your plan but all plans would at least require an additional global option to be added if you want to use the device in Italy. Without a plan charges would be per use and while the cost wouldn't be specifically for roaming it could add up quickly.
