Undergrounding Communication Line

Hello,

I'm an engineer working on a project in Montclair, CA where there is existing aerial/overhead Verizon line mounted on utility (Southern California Edison) poles that we will need to convert into underground installation. Some or all poles are to be removed if possible. Who should I contact (email and phone) regarding this process and is there a form I should fill out to trigger this process?
