Compliment to employee

Question

I am a Senior not used to modern technology, I visited the Verizon store in Doral, Florida, and was assisted by a wonderful person who was patient, understanding and sweet; she took two hours to make sure that I left with the device I really need and trained in its use. She deserves a special recognition. I read that there is a form for this purpose, but I cannot find it online. Any help will be appreciated. Thank you.
