Biling Issue Still Not Resolved After Five Months

I have been trying five months to resolve a billing issue related to a promotional credit being inadvertently removed. During each of the five phone calls I can tell they are reading the same rough script (I'm very sorry - I promise to resolve it this time ...) and promise to get back to me with an update in five days. But nothing has gotten resolved
What do I do to get someone to actually solve the problem? How do I escalate?
