Verizon LTE Network Extender Server Error 5A - How to Register MAC Address with Verizon

I have Verizon Fios as well as Verizon Wireless cell coverage, and I purchased a Verizon LTE Network Extender to improve cell service in my home.

Easy enough to connect but getting Server Error 5A, which apparently means means the extender has failed to communicate with Verizon's Certificate Management Platform (CMP) server.

Have spent three hours on customer service getting bounced back and forth between Verizon Fios and Verizon Wireless; Wireless thinks it must be a Fios problem and vice versa.

Helpful comments here indicate that the issue is that the MAC address for the booster unit needs to be registered with Verizon, which makes sense, and those who have done that have successfully resolved the issue.

Does anyone know how to actually have Verizon add the MAC address for the booster which apparently makes this work?  Any idea if it is something that Verizon Wireless does or that Verizon Fios does?  Thank you!

My understanding was the 4G Network Extenders required a BROADBAND (cable) internet connection and not satellite nor Fios. Some users have said theirs DID work with Verizon Fios but we haven't heard anything definitive from Verizon. Does your network extender show up on your Devices list on MyVerizon? You might need to use a browser and not the MyVerizon app to see as the app does not show the extender devices (at least not on my moto z4).  The knowledgeable VZW tech support agents (level 2/3) know how to add the MAC address to the appropriate database.
